Only the third example, & the only unused example, we have seen of this rare provisional issue of Postal Stationery prepared specifically for mailing phone card orders. Ex 'Tillie Mitchell'. [The two used examples sold at the Prestige auctions of 8.11.2013 & 30.9.2015 for $437 & $145 (a gift!) respectively]
